Transition of Care (TOC) Program
Partnering with Arizona hospitals, Blue Cross Blue Shield of Arizona launched the Transition of Care (TOC) program in January 2011 to lower hospital readmission rates for its members.
TOC aims to decrease these rates by empowering members:
- Fill their prescriptions after discharge from hospital
- Keep follow-up appointments with their physicians
Exercising these two key measures can avoid millions of dollars in healthcare costs each year. This year, the TOC program will save an estimated $2 million in claims cost! By simply focusing on these two evidence-based measures, we can collectively improve patient outcomes and reduce healthcare costs.
